You may receive some
assistance already but are finding it difficult to manage. You may be
thinking about moving into a hostel or nursing home simply because you can
no longer cope with everyday tasks. Your carer may be finding it difficult
to care for you at home. Great Lakes Community Options Project can help
you to stay living at home. Older people who are frail, people with
disabilities and carers are eligible for Community Options assistance if
they are having difficulty coping at home and need additional services and
support to continue living at home.
What is Community Options?
Community Options is a client oriented service which aims to provide
flexible responses to your needs. This is achieved by co-ordination and
case management across service boundaries to provide support,
encouragement, monitoring and advocacy as required.
Great Lakes Community Options
Project is under the auspices of Great Lakes Council and receives funding from the
Commonwealth and State Governments through the Home and Community Care (HACC)
Program. The project has been in operation since August 1990. There are
three part time staff members:
Community Options is part of Great Lakes Council’s Community Services
Branch which is managed by Andrew Braybrook. Older people who are frail, people
with disabilities and carers who live in the Great Lakes Local Government Area
can apply for assistance. Great Lakes is located on the Mid North Coast of New
South Wales in Australia.
40 clients receive varying assistance from Great Lakes Community Options
Project, depending on their needs. Clients range in age from 0-95 years old.
Some of the ways Great Lakes Community Options Project has assisted clients to
stay living in their own home include:

organising transport and a volunteer to go shopping

connecting the telephone

supplying a personal alarm system for an older client to use in an
emergency

organising weekly respite to give a carer of a person (including a person
with dementia and/or challenging behaviours) a break

assisting with preparing an evening meal
|
 |
giving an older client who lives on her own a pet dog
|
 |
organising a holiday at Port Macquarie for a client
|
 |
supplying a wheelchair to an older client
|
 |
organising regular physiotherapy for a client with a disability
|
 |
assisting a carer of a person with a disability to attend TAFE.
